Personal information that we collect from you includes: (i) Contact Information such as name, email address, mailing address, phone number, and (ii) Billing Information such as credit card number, and billing address.  It is possible in connection with certain health care related services that we might obtain access to certain identifiable personal health information (“PHI”). To the extent we obtain access to your PHI, we will maintain the confidentiality of the PHI in accordance with all applicable laws and our contractual obligations.
